ASHLink Programs
Active & Fit - Standard membership dues are covered by Active & Fit. Contact your insurance plan to obtain fitness ID number/confirmation code.

Silver & Fit - Standard membership dues are covered by Silver & Fit. Contact your insurance plan to obtain your fitness ID number/confirmation code.

NIHCA
NIHCA - Offers a $20.00 reimbursement if you meet your plan's monthly facility check in goal. Register for these reimbursements at nihcarewards.org. Please alert Member Services you are a NIHCA participant.
Optum/Healthy Contributions Programs
One Pass - Standard membership dues are covered by One Pass (Optum). A fitness ID number is required for a One-Pass membership. Click here to verify eligibility and obtain your fitness ID number/confirmation code.

Prime Fitness Programs
Tivity Health - Standard membership dues are covered by Tivity Health. A Tivity Health card or fitness ID number is required for a Tivity Membership.
Well On Target - Standard membership dues are covered by Well On Target. A Well On Target Health card or fitness ID number is required for a Well On Target membership.
Fitness Your Way - Standard membership dues are covered by Fitness Your Way. A Fitness Your Way card or fitness ID number is required for a Fitness Your Way membership
Self-Reporting Programs
Quartz - Self-reporting program. Quartz will provide members with a prepaid card that can be used to pay membership dues
Insurance-based memberships may not be eligible for Nationwide Membership. Please check with your insurance-based program to find out how to gain multiple facility access.
Not every insurance plan is eligible for a membership benefit. You must call the number on the back of the card to determine eligibility and program criteria. Some programs require program registration before a membership is opened. Member Services staff is unable to obtain coverage information on behalf of members.
